Waist circumference is a simple measurement with a tape that indirectly reflects the amount of dangerous visceral fat around the internal organs. That is why the waist is often more informative than weight and BMI: a person with a "normal" weight but a large belly still falls into the higher-risk zone.
An enlarged waist is a key sign of abdominal obesity and one of the criteria for metabolic syndrome. It is conveniently complemented by derived measures — the waist-to-height ratio and WHR.
Why the waist matters more than the number on the scale
Weight and BMI do not distinguish where the fat lies. But risk to the heart and metabolism is created primarily by fat in the abdominal area: it surrounds the liver, pancreas and intestines, actively interferes with metabolism and sustains chronic inflammation.
Waist circumference is the most accessible way to suspect an excess of such fat without complex equipment. At the same weight, a person with an "apple" (android) fat distribution is at greater risk than a person with a "pear" shape.

The threshold values differ by sex and ethnicity. Below are the WHO/IDF guides for people of European descent; for people of South and East Asian descent the thresholds are lower.

How the test is done
Measure in the morning on an empty stomach, standing, with the waist unclothed. Place the tape horizontally midway between the lower edge of the ribs and the top of the hip bone (roughly at the level of the navel), after a normal exhalation, without pulling the belly in. The tape lies snugly but does not compress the skin.
What affects the value
- Overeating and bloating — measure on an empty stomach.
- Pulling the belly in understates the result — breathe calmly.
- Tape level: measuring at the widest point of the belly instead of the standard point overstates the figure.
- Pregnancy, ascites — the measure does not reflect fat.
