What metabolic syndrome is
Metabolic syndrome is not a disease in its own right but a symptom complex: a group of metabolic, hormonal and clinical disturbances that together sharply raise the risk of heart disease, diabetes and stroke. The key link is visceral obesity, and at the metabolic core is the tissues' unresponsiveness to insulin — insulin resistance.
The insidious part is that each individual marker may be only “slightly above normal” and cause no concern. But the factors reinforce one another, creating a synergistic effect that is more dangerous than any of them alone. That is why early detection — before a serious health event — is one of the most effective steps for long-term health.
How common is it? According to the WHO, metabolic syndrome is found in 20–40% of the world's population, more often in middle-aged and older people (30–40%). Population studies find that around 40% of people have 2 components of the syndrome and 11% have 3 or more. Prevalence is especially high in at-risk groups: among people with obesity it reaches 49%, with impaired glucose tolerance 50%, and with diabetes 80%.
The five criteria: how the diagnosis is made
Under the harmonized IDF/AHA-NHLBI 2009 definition, metabolic syndrome is diagnosed when any 3 of 5 criteria are present — one of them concerns blood pressure. Taking the relevant medication (for blood pressure, or to lower lipids or blood sugar) counts the corresponding criterion regardless of the current value.

How to measure your waist correctly
Accuracy depends on technique. Measure under the same conditions each time, ideally in the morning. Per the WHO protocol, waist circumference is measured at the midpoint between the lower edge of the last palpable rib and the top of the iliac crest, with the tape parallel to the floor, not compressing the skin, taken on a relaxed exhale. Along with the waist, it is useful to assess your body mass index and waist-based risk.

Causes and risk factors
Most often the root cause is insulin resistance: cells respond less well to insulin, and blood sugar rises. Understanding your personal risk factors lets you intervene early.

Metabolic syndrome often develops quietly, but possible signs do exist. Regular check-ups are important to catch it before complications arise.
Metabolic syndrome and age
The frequency of the syndrome gradually rises with age — in both men and women — peaking in the 60–69 age group, where nearly one in four adults has this symptom complex. In old age the rise seems to “plateau” by around 74. Today metabolic syndrome is included among age-associated conditions.
Even more troubling is the syndrome's shift to younger ages: over the past 20 years the share of children aged 6–11 who are overweight has grown from 5 to 17%. It is most noticeable in large cities — a consequence of fast food, low physical activity and screen-based leisure.
Why metabolic syndrome is dangerous
Having the syndrome substantially raises the risk of serious disease. People with metabolic syndrome are twice as likely to develop cardiovascular disease and five times as likely to develop type 2 diabetes. The combination of factors creates a synergy more dangerous than any single factor alone.
Prevention: what's in your hands
The good news is that almost every link of the syndrome is manageable, and lifestyle changes work on all five criteria at once.
- Losing weight and waist. Losing 5–10% of body weight noticeably reduces visceral fat — the main driver of the syndrome.
- Movement. 150 minutes of moderate aerobic activity per week plus strength training raises insulin sensitivity and boosts HDL.
- Nutrition. Less sugar, fast carbs and processed foods; more vegetables, fiber, whole grains and lean protein (the DASH / Mediterranean pattern).
- Sleep and stress. Sleep deprivation and chronic stress worsen carbohydrate metabolism and blood pressure.
- Regular screening. Early detection through lab tests and measurements is key to preventing complications.
