Waist-to-height ratio (WtHR)

The WtHR (waist-to-height ratio) is the ratio of waist circumference to height. It specifically assesses abdominal obesity — fat around the belly that is closely linked to heart disease and diabetes. Many specialists consider WtHR more accurate than BMI: it doesn't "penalize" muscular people and it catches the dangerous "thin outside, fat inside" type.

What WtHR shows and why it beats BMI

BMI accounts only for weight and height and therefore often gets it wrong: it can show "obesity" in an athlete with a large muscle mass, or "normal" in a person with thin arms and legs but a dangerously large belly (the skinny fat type). WtHR looks precisely at the waist relative to height, and therefore at the visceral fat around the organs that determines cardiometabolic risk. That's why WtHR works well for people of different heights, ages and builds.

How to measure correctly

The key thing to remember: your waist circumference should be less than half your height (WtHR < 0.5). This simple rule is easy to check even without a calculator, it's clear to children and adults alike, and it needs no tables. If your waist has already passed half your height, that's a signal to tackle abdominal fat.

WtHR, BMI, WHR or body-fat % — which to choose

The metrics complement each other, each telling you something different:

  • WtHR — abdominal fat relative to height (more accurate than BMI on "belly fat").
  • WHR — the type of fat distribution (an "apple/pear" body shape).
  • BMI — the ratio of weight to height (a rough weight screen).
  • Body-fat percentagehow much fat and muscle the body holds.

Why is WtHR more accurate than BMI?

BMI doesn't distinguish muscle from fat and can't see where fat is stored. WtHR looks at the waist — a direct marker of abdominal (visceral) fat, which is what determines cardiometabolic risk. That's why it errs less often in muscular people and in people with a "hidden belly".

What WtHR is considered normal?
