Triglycerides (TG) are the main form in which fat is stored and transported in the body. Some come from food, and some are made by the liver — primarily from an excess of carbohydrates, not from fat, as many people think.
High triglycerides almost always go hand in hand with low HDL. This combination is called atherogenic dyslipidemia — a characteristic sign of insulin resistance and visceral obesity.

What an elevated level means
Elevated triglycerides are an independent cardiovascular risk factor and one of the five criteria of metabolic syndrome.
Common causes: excess refined carbohydrates and sugar, alcohol (even moderate amounts raise TG sharply), visceral obesity, insulin resistance and type 2 diabetes, hypothyroidism, kidney disease, and certain medications.
Separately: very high values (above 10 mmol/L) are dangerous less for the vessels than for the risk of acute pancreatitis.
What a low level means
Low triglycerides are not considered a problem in themselves and usually indicate a favorable lipid profile.
Very low values occasionally occur with hyperthyroidism, malabsorption syndrome, severe nutritional deficiency or against the background of some rare genetic conditions — in that context they are assessed together with the other markers.
How the test is done
Triglycerides are the most "capricious" marker of the lipid panel: they depend on food more than the others. Blood is drawn strictly fasting, 12 hours without food (water is fine).
For 2–3 days before the test avoid alcohol: it can raise TG several-fold and completely distort the picture. It is also worth avoiding unusually fatty meals and heavy workouts the day before.
What affects the value
- Refined carbohydrates and sugar — the main dietary driver: the liver turns their excess into triglycerides.
- Alcohol — one of the most powerful factors that raise them.
- Visceral fat and insulin resistance — push TG up and HDL down.
- Omega-3 fatty acids — proven to lower triglyceride levels.
- Regular aerobic exercise and weight loss.
- Hypothyroidism — untreated, it raises TG; with high TG it makes sense to check TSH.

My triglycerides are high — how dangerous is that?
A moderate rise (1.7–2.25) is a signal to review your diet and alcohol; it rarely needs medication by itself. Values above 5.6 mmol/L need a doctor, and above 10 this is a risk of acute pancreatitis and an emergency.
How quickly can triglycerides be lowered?
This is one of the most responsive markers. Quitting alcohol and sharply cutting sugar and refined carbohydrates can noticeably lower the level within 2–4 weeks. Add aerobic exercise and omega-3.
Why do you have to test strictly fasting?
Triglycerides rise physiologically after eating — a non-fasting measurement gives an inflated result and a false alarm. That is why the lipid panel requires 12 hours of fasting, whereas 8 is enough for many other tests.
