Metabolic Index (MI)
Insulin resistance is an early metabolic disturbance that precedes diabetes, atherosclerosis and heart disease by years. The metabolic index gives it a quantitative estimate from three routine lab tests — triglycerides, glucose and HDL. Its strength is early sensitivity: the index rises at the very first metabolic shifts, while blood sugar is still normal.
What the metabolic index is
In insulin resistance, cells in fat, muscle and liver tissue respond poorly to insulin, which disrupts glucose metabolism and raises blood sugar. IR is a predictor of obesity, type 2 diabetes and atherosclerosis, as well as an independent risk factor for vascular events. That is why detecting it early is a method of primary prevention for cardiovascular disease and diabetes. The metabolic index gives a qualitative and quantitative assessment of IR and changes even with isolated early shifts in carbohydrate and lipid metabolism.

What raises the metabolic index
- High triglycerides and low HDL — atherogenic dyslipidemia, an early companion of insulin resistance (the index is especially sensitive to it).
- Elevated fasting glucose — even within the “high-normal” range.
- Visceral fat and excess weight — directly increase insulin resistance. To assess visceral fat, try the LAP index.
- A sedentary lifestyle and a diet high in fast carbohydrates — constant spikes of glucose and insulin.

Frequently asked questions
How does the metabolic index differ from METS-IR?
Both assess insulin resistance without an insulin test. The metabolic index is calculated from just three blood tests (TG, glucose, HDL) and is sensitive to early metabolic shifts. METS-IR additionally factors in BMI and is tuned to predict type 2 diabetes.
Why enter your sex if it is not part of the formula?
Sex does not affect the index itself, but it is needed to assess HDL correctly: the normal level is above 1.0 mmol/L for men and above 1.2 mmol/L for women.
Does a high index already mean diabetes?
No. A high MI points to insulin resistance and elevated risk, but it is not a diagnosis. It signals that you should adjust your lifestyle and possibly get checked. A physician diagnoses prediabetes or diabetes based on fasting glucose, HbA1c or a glucose tolerance test.
How often should you recalculate?
