Mean arterial pressure — the perfusion of organs with blood

Mean arterial pressure (MAP) is the averaged pressure in the arteries over the whole cardiac cycle. It is what determines whether the organs are supplied with enough blood.

It is calculated by the formula MAP ≈ diastolic + ⅓ (systolic − diastolic): since the heart spends longer in diastole, the lower number contributes more.

Important: MAP is especially important in intensive care, where it is kept above 65 mmHg for an adequate blood supply to the brain, heart and kidneys.

What an elevated level means

Elevated MAP accompanies arterial hypertension and means a chronic load on the vessels and target organs. It raises the risk of damage to the heart, brain and kidneys, as does high blood pressure in general.

What a low level means

Low MAP is a more alarming signal than simply a low "upper" number, because it directly reflects an inadequate blood supply to the organs.

A MAP below 60–65 mmHg threatens the perfusion of the brain, heart and kidneys. This occurs with shock, severe blood loss, sepsis, dehydration — and requires emergency care.

How the test is done

MAP is usually not measured directly (except with invasive monitoring in intensive care) but calculated from systolic and diastolic pressure. Many monitors display it automatically.

In everyday life the usual upper and lower numbers are the guide; MAP is more often used by doctors in emergencies and in intensive care.

What affects the value

  • The overall BP level — determines MAP directly.
  • Blood volume (blood loss, dehydration) — lower it.
  • Vascular tone (sepsis, shock) — sharply lowers it.
  • The work of the heart — with weak ejection MAP falls.
  • Antihypertensive medications — lower it.
  • Stress and exertion — temporarily raise it.

Why is mean arterial pressure needed?

It reflects the real blood supply to the organs over the whole cardiac cycle more accurately than the upper or lower number alone. That is why MAP is especially important in intensive care, where it is kept above 65 mmHg to protect the brain, heart and kidneys.

How do you calculate MAP?

By the formula: diastolic plus a third of the difference between systolic and diastolic. For example, at 120/80 it is 80 + (40/3) ≈ 93 mmHg. Many electronic monitors display MAP automatically.

What MAP is considered dangerously low?

Below roughly 60–65 mmHg the blood supply to the organs becomes inadequate. Such a MAP occurs with shock, blood loss and sepsis and requires emergency medical care.
