What blood pressure is

Blood pressure (BP) is expressed as two numbers. The upper one (systolic, SBP) is the pressure at the moment the heart contracts, on average 100–120 mmHg. The lower one (diastolic, DBP) is measured as the heart relaxes; it reflects the elasticity of the vessels and averages 60–80 mmHg. The benchmark is 120/80; a sustained rise above 140/90 is called hypertension.

Both numbers independently affect cardiovascular risk. The category is set from the average of 2–3 resting measurements; if SBP and DBP fall into different categories, the higher one is used. For a precise assessment and an extended masked-hypertension calculator, see the masked hypertension tool.

Blood pressure classification (ACC/AHA)

The category is set by the higher of the two numbers. Each step means both a rise in cardiovascular risk and a contribution to the biological age of your vessels:

Pulse and mean pressure

Pulse pressure (PP) is the difference between SBP and DBP (normal 40–60 mmHg). A narrow value (< 40) may point to reduced cardiac contractility; a wide one (> 60) points to arterial stiffness: every +10 mmHg raises the risk of coronary heart disease by about 23%. PP naturally rises with age as the arteries lose elasticity.

Mean arterial pressure (MAP) = DBP + ⅓ × PP (normal 70–100 mmHg) is the average force of blood against the vessel walls over the cardiac cycle and a key measure of organ blood supply. Keeping MAP ≥ 65 mmHg is critical to prevent hypoperfusion.

Average BP rises with age because of changes in arterial elasticity and lifestyle, but this is not inevitable. Reference average pressures (AHA, 2017):

*In older adults, slightly higher targets are sometimes allowed (up to 140/90 with frailty) to avoid dizziness and falls from overly aggressive treatment. In women, blood pressure often rises during menopause as estrogen declines.

Blood pressure and biological age

The link between blood pressure and vascular age is very close. On average, in men with hypertension the biological age exceeds the chronological one by 17.6 years, and in women by 13.4 years. The lower the pressure and the closer the resting heart rate is to 60 bpm, the "younger" the cardiovascular system. Why blood pressure changes with age and beyond

  • Arterial stiffness. Elastic fibers lose flexibility from aging, sodium, smoking and inflammation — it becomes harder for the heart to push blood, and SBP rises.
  • Circadian rhythm. Blood pressure peaks in the morning (6–12 a.m.) alongside cortisol and is lowest during sleep — measure at the same time each day.
  • The "white-coat" effect. A temporary rise of more than 20/10 mmHg at the doctor's office — home monitoring is more accurate.
  • Hormones. In women, estrogen keeps the vessels flexible; during menopause its decline often causes a jump in blood pressure.

What blood pressure is considered normal?

For an adult at any age, below 120/80 mmHg. Values of 120–129/80–84 are still normal but already a reason to keep an eye on it; 130–139/85–89 is high-normal.

How do you measure blood pressure correctly at home?

For 30 minutes beforehand: no coffee, smoking or exertion; sit and rest for 5 minutes; keep the cuff at heart level; don't talk. Take 2–3 measurements a minute apart and use the average, ideally in the morning and evening over several days.

The risk score is high — what should I do?

The score sums up your blood pressure and lifestyle factors (weight, activity, sodium, alcohol, stress and others). A high score is a reason to discuss testing with your doctor and to start with the factors you can control: the DASH diet, less salt, movement, weight management.
