Masked (hidden) hypertension

You can feel perfectly fine while, for years, running a blood pressure that quietly damages the heart, brain, kidneys and vessels. Masked hypertension is when blood pressure is normal in the doctor's office, but elevated outside the clinic. It is linked to roughly a twofold rise in the risk of heart attack and stroke. What blood pressure is

Blood pressure is the force of blood against the vessel walls. It is expressed as two numbers:

  • Upper (systolic, SBP) — the pressure at the moment the heart contracts hardest. It depends on the work of the left ventricle and the blood volume. On average 100–120 mmHg.
  • Lower (diastolic, DBP) — the pressure at the moment the heart relaxes. It reflects the tension and elasticity of the artery walls. On average 60–80 mmHg.

For example, 120/80 mmHg (“120 over 80”) is considered the normal benchmark. Both values independently affect cardiovascular risk. The difference between them is called the pulse pressure — it grows with age and vascular stiffness.

Blood pressure follows a circadian rhythm: it peaks in the morning (between 6:00 and noon), dips during the day, rises slightly in the evening and bottoms out during sleep. That is why it is worth measuring at the same time each day.

Why masked hypertension is so insidious

High blood pressure usually causes no pain and gives no obvious signals — which is why it is called a “silent” enemy. Many people learn about their hypertension only after a serious complication.

Masked hypertension is suspected in people with high-normal blood pressure: 130–139 / 85–89 mmHg. Formally this is still the “upper limit of normal,” but it is exactly here that blood pressure is often elevated at work, under stress or during sleep — quietly damaging the organs. Its opposite is the white-coat effect, when blood pressure spikes only in the doctor's office. Home and 24-hour monitoring help tell the two apart.

Predisposing factors for masked hypertension: young age, diabetes, coronary heart disease, kidney disease, sleep apnea, smoking, alcohol and a sedentary lifestyle.

Why high blood pressure is dangerous

High blood pressure responds to both prevention and treatment, yet when left uncontrolled it claims more than 10 million lives every year. There are two types: primary (essential) hypertension — 90–95% of cases, tied to genetics and lifestyle, which is controlled rather than “cured”; and secondary — a consequence of other conditions (kidney or endocrine disease) that can resolve once the underlying cause is treated.

Blood pressure and biological age

The link between vascular age and blood pressure is very tight. On average, men with hypertension have a biological age that exceeds their chronological age by 17.6 years, and women by 13.4 years. The higher the systolic pressure, the wider the gap, especially in younger age groups. That makes blood pressure control one of the most effective ways to slow vascular aging, and you can gauge it directly with the vascular age.

An important nuance: in older people (over 80–85) and those whose biological age noticeably outpaces their chronological age, a diastolic pressure that is too low — below 70 mmHg — is also unfavorable. On one hand, it lowers perfusion pressure in the myocardium and worsens its ischemia; on the other, it reflects a rise in pulse pressure and increased vascular stiffness. In other words, both extremes are harmful for vascular aging.

How to measure blood pressure correctly

  • For 30 minutes before measuring — no smoking, coffee or physical exertion.
  • Before measuring — 5–10 minutes of seated rest, in quiet.
  • Seated at a table, back supported, feet on the floor, legs uncrossed.
  • Arm relaxed on the table, cuff on the upper arm — at heart level, 2–3 cm above the elbow.
  • During the measurement, do not move or talk.
  • Take a series: at least 2–3 readings a minute apart; for diagnosis — morning and evening over 3–7 days.
