5-year cardiovascular risk is the probability of developing a cardiovascular event (heart attack, stroke or cardiac death) within the next five years. This is a shorter forecast horizon than the familiar 10-year risk.
A short window is useful where a ten-year estimate is uninformative: in older people, in those already at high risk, or when a "here and now" decision matters.
5 or 10 years — what's the difference
Most scales (Framingham, SCORE2, QRISK, Pooled Cohort Equations) calculate risk over 10 years by default. The five-year risk is roughly half the ten-year risk for the same factor profile, because fewer events accumulate over a shorter period.
A short horizon is more meaningful for older people: for a 75-year-old the 10-year risk is almost always "high" simply because of age, while a 5-year estimate better reflects the actual near-term outlook.
How to interpret the number
There are no unified international thresholds specifically for 5 years — they are adapted from the ten-year ones. As a rough guide a risk up to ~2.5% is considered low, 2.5–5% borderline, 5–10% high, and above 10% very high. Exact values depend on the chosen model and region.
As with the 10-year risk, some factors are modifiable: blood pressure, lipids, smoking, diabetes. Correcting them shifts the short-term forecast too.
