TIBC (total iron-binding capacity) shows how much iron the blood can theoretically bind — that is, the total "capacity" of transport. In essence it is an indirect measure of transferrin.
Like transferrin, TIBC behaves as a mirror of iron stores: little iron — capacity rises (many free "seats"); much iron — it falls. So TIBC is looked at with iron, ferritin and transferrin saturation.

What an elevated level means
Elevated TIBC means there are many "free seats" for iron in the blood — a typical picture of iron deficiency. It is usually accompanied by low iron, ferritin and transferrin saturation.
Physiologically, TIBC rises in pregnancy.
What a low level means
Reduced TIBC means "few seats". This occurs with iron overload (hemochromatosis), as well as with conditions that lower transferrin: inflammation and chronic disease, liver disease, protein deficiency.
To understand exactly what is happening, TIBC is always interpreted together with ferritin and transferrin saturation.
How the test is done
TIBC is measured from venous blood, ideally in the morning fasting. On its own it is not very informative — its value lies within the iron profile (iron, ferritin, TIBC/transferrin, saturation), where the markers complement each other.
What affects the value
- Iron deficiency — raises TIBC.
- Iron overload — lowers it.
- Inflammation and chronic disease — lower it.
- Liver disease and protein deficiency — lower it.
- Pregnancy, estrogen — raise it.
