Subclinical conditions — an iceberg with a hidden underwater part

Subclinical conditions are abnormalities that are already noticeable in tests or examinations but do not yet show as complaints and symptoms. The word "subclinical" literally means "below the threshold of clinical manifestations".

Such conditions are an important target of preventive medicine: they are found during health screening and laboratory tests, and early intervention helps prevent progression to overt disease.

Typical examples

Subclinical hypothyroidism — elevated TSH with normal free T4. Prediabetes — blood sugar above normal but not yet diabetes. Masked hypertension — normal blood pressure at the clinic and elevated at home.

This also includes subclinical atherosclerosis (plaques without symptoms), as well as mild hypocalcemia and early hyperparathyroidism found on tests.

Why it matters to find them early

Disease often develops over years without symptoms. At the subclinical stage, lifestyle alone — diet, movement, weight loss — is often enough to stop or reverse the process, and early treatment reduces the risk of complications.

At the same time, not every abnormality needs treatment: it is important not to "overdiagnose" and not to treat a number instead of a person. The decision is made by weighing risks and benefits.

What to do about a finding

First — confirm it: repeat the test, since a single deviation can be random. Then assess risk factors and, if needed, monitor over time. The approach (observation or treatment) is decided by a doctor, taking into account age, comorbidities and overall risk.

Important: One deviation is not yet a diagnosis: it is confirmed on repeat testing and interpreted in the light of risk factors.

Does "subclinical" mean not dangerous?

No. It means "still without symptoms". The disorder itself is already present and over time may progress into overt disease. But not every subclinical deviation needs treatment — often observation and lifestyle changes are enough.

How are subclinical conditions detected?

Mainly by screening people with no complaints: laboratory tests (TSH, glucose, HbA1c, lipids), blood-pressure measurement, imaging studies. This is exactly why preventive check-ups and health screening exist.
