MSCV (Mean Sphered Corpuscular Volume) is a parameter measured by some hematology analyzers after the red blood cells are artificially brought to a spherical shape. In value MSCV is close to the usual MCV, but it is sensitive to the difference between them.
MSCV is an auxiliary, largely research parameter. Its main value is helping to distinguish the causes of microcytosis: iron deficiency and thalassemia.

There is no single universal reference — MSCV is assessed in comparison with the MCV of the same test (the unit is the femtoliter, fL):

What an elevated level means
If MSCV is higher than or equal to MCV in microcytosis, this more often points to iron-deficiency anemia: the red cells are small but behave "normally" when sphered. Such a result is a reason to check ferritin and transferrin saturation.
What a low level means
MSCV markedly lower than MCV (roughly below ~78–80 fL with normal MCV) is characteristic of thalassemia carriage: such red cells have an altered membrane and shape. This helps not to miss a hereditary cause of microcytosis when iron is normal.
How the test is done
MSCV is calculated automatically during a regular complete blood count on a compatible analyzer — nothing separate needs to be tested. The parameter is interpreted only together with MCV, RBC, RDW and iron stores; MSCV is not used in isolation.
What affects the value
- The analyzer model and its calibration — the parameter is poorly standardized between devices.
- Mixed conditions (iron deficiency together with thalassemia) erase the difference between MSCV and MCV.
- Storage and sample quality affect the shape of the red cells and the result.

Why is MSCV needed if there is MCV?
MCV shows the average volume of red blood cells, but in microcytosis it does not distinguish iron deficiency from thalassemia. Comparing MSCV and MCV increases accuracy: in thalassemia MSCV is usually lower than MCV, in iron deficiency it is not. It is a cheap screening sign that does not replace a genetic test.
Is there an MSCV in my test?
Not always. MSCV is reported only by some analyzers (chiefly Beckman Coulter), so most ordinary reports do not have it. If the parameter is absent, RDW, ferritin and hemoglobin electrophoresis are used to distinguish the causes of microcytosis.
