Liver Stiffness Score (LSS)

LSS (Liver Stiffness Score) is a calculated model that uses routine blood tests to estimate liver tissue stiffness and the likelihood of fibrosis. The further fibrosis progresses, the denser (stiffer) the tissue becomes. LSS is a calculated alternative to elastography: according to its authors the model outperforms FIB-4, APRI and HSI.

What liver stiffness and LSS are

Liver stiffness is the firmness of its tissue, linked to scarring. Stiff areas in the liver are a sign of fibrosis, which over time can lead to cirrhosis. The further fibrosis progresses, the denser the tissue becomes — which is why measuring stiffness helps to judge the stage of the process. Detecting fibrosis early makes it possible to adjust lifestyle and treatment in time.

LSS (Liver Stiffness Score) is a calculated estimate of liver tissue stiffness. It is currently one of the best models for predicting liver fibrosis from simple laboratory data: according to its authors it outperforms FIB-4, APRI and HSI. In essence, LSS tries to reproduce the result of elastography (FibroScan) where the device is not available.

A healthy liver is soft and elastic. As it scars, it hardens. LSS estimates that firmness from blood tests and turns it into an easy-to-read risk scale: a low result is reassuring, a high one is a reason to get an elastography and see a physician.

Liver fibrosis: from fat to scars

Liver fibrosis is the gradual replacement of healthy liver tissue by connective (scar) tissue, which disrupts the organ's structure and function. Where steatosis indices answer the question “is there fat in the liver,” LSS answers the next one: “has the process turned into scarring, and how dense has the tissue become.” In NAFLD and other chronic liver diseases, it is the fibrosis stage that largely determines the prognosis.

Fibrosis develops in stages (F0→F4 on the METAVIR scale): the further the process has gone, the stiffer the tissue. LSS estimates which side of that path you are more likely on.

What's needed for the calculation

LSS is calculated from six values — two about you and four from routine blood tests:

  • Body weight, kg. It enters the calculation directly; enter your current weight.
  • Race. The model accounts for race (a separate adjustment is applied for Black race).
  • Fasting glucose, mmol/L. It sets the diabetes flag: a value ≥ 7.0 mmol/L is treated as diabetes mellitus. If you have been diagnosed with diabetes, enter 8.0 mmol/L.
  • AST, U/L. A liver enzyme; it rises when liver cells are damaged.
  • Platelet count, ×10⁹/L. As fibrosis progresses their number usually falls.
  • Albumin, g/L. A protein synthesized by the liver; it falls with severe liver damage.

How to prepare properly

Weight

Accuracy depends on measurement technique. Take your measurements under the same conditions: on the same scale, ideally in the morning and on an empty stomach.

Blood tests

AST, albumin and glucose are taken on an empty stomach; platelets come from a complete (clinical) blood count in units of ×10⁹/L. The day before, avoid intense workouts and alcohol — they temporarily raise the enzymes and will distort the result.

These are nested probabilities: each shows the chance that fibrosis has reached at least the stated stage (F4 is included in F3–F4, and F3–F4 in F2–F4). That is why they don't add up to 100% and normally decrease from top to bottom.

How to read the result

The LSS result is a number that grows the higher the estimated tissue stiffness and the likelihood of fibrosis. The zones are interpreted as follows:

Fibrosis stage probabilities

Beyond the overall risk zone, LSS lets you estimate individual probabilities on the METAVIR scale:

  • F2–F4 — significant fibrosis. The likelihood that fibrosis has reached a clinically significant stage (F2 and above).
  • F3–F4 — advanced fibrosis. The likelihood of advanced fibrosis that calls for active monitoring.
  • F4 — cirrhosis. The likelihood that the process has reached cirrhosis.

These are exactly probabilities, not a verdict: they help you understand how persistently you should pursue instrumental confirmation.

Why these particular values

The model combines features that change in different ways as fibrosis develops: the enzyme AST reflects damage to hepatocytes, while falling platelets and albumin are indirect signs that liver disease is progressing, and weight, race and diabetes capture the metabolic background. Glucose is needed in the model to determine the presence of diabetes — a significant driver of NAFLD progression.

Its place in the section and other indices

LSS is about stiffness and fibrosis — that is, a later stage than fat accumulation. A logical pairing: FLI, HSI and NAFLD-LFS assess steatosis, FIB-4 and FibroTest assess fibrosis with other formulas, while LSS adds a stiffness estimate as a calculated substitute for elastography. It is convenient to compare several indices with one another, and the conclusion is always interpreted by a physician.

Limitations and important caveats

  • It is not a diagnosis. LSS estimates the likelihood of fibrosis, which is confirmed by elastometry/elastography, ELF or biopsy.
  • It is an estimate, not a measurement. Actual stiffness is measured by a device (FibroScan/VCTE); LSS only predicts it from lab tests.
  • It depends on data quality. Errors in weight, AST, platelets, albumin or glucose shift the result.
  • The intermediate zone is a common outcome. It is not an error but a signal that additional methods are needed.
  • It does not replace an examination. The result is interpreted by a physician, taking your history and comorbidities into account.

Frequently asked questions

How is LSS different from FibroScan?

FibroScan measures liver stiffness with a device. LSS is a calculated model that predicts stiffness from blood tests when the device is not available. With a high LSS, elastography is still worth doing.

What do I enter in the glucose field if I have diabetes?

Enter 8.0 mmol/L — the model will treat this as diabetes being present. In general, a value ≥ 7.0 mmol/L is treated by the calculator as diabetes.

Where do I find albumin, and in what units do I enter it?

Albumin is on a blood chemistry panel. Enter the value in g/L (as on the report) — the calculator will convert it to the required units itself.

Does a high LSS already mean cirrhosis?

No. A high result only increases the likelihood of advanced fibrosis and cirrhosis and requires confirmation by instrumental methods. The diagnosis is made by a physician.

What should I do with the result?

Show it to your physician along with your test results. For a low result — monitoring and work on risk factors; for an intermediate or high one — elastography and a work-up plan.
