Sarcopenia — age-related loss of muscle fibers

Sarcopenia is the age-related loss of muscle mass, strength and function. After 30–40, muscle begins to decline slowly, and without exercise this process speeds up.

This is not cosmetics but a matter of longevity: muscle is support, metabolism and protection against falls. Preserving muscle mass is one of the most underrated factors in healthy aging.

Reference ranges

Sarcopenia is assessed by three components:

Strengthreduced grip strength, difficulty rising from a chair
Masslow muscle mass index (SMI/FFMI)
Functionslower walking, unsteadiness
Important: There is no single "blood test for sarcopenia". The diagnosis is built on strength, muscle mass and function. As a guide to loss: after 50, muscle declines by roughly 1–2% per year, and strength faster.

What an elevated level means

Here a "rise" is, conversely, good muscle mass and strength. What is worrying is their decline.

What speeds up sarcopenia: inactivity and bed rest, protein deficiency in the diet, chronic diseases and inflammation, a decline in sex hormones, a lack of vitamin D.

Why it is dangerous: falls and fractures, loss of independence, worsening metabolism, and a harder course of any illness.

What a low level means

Low muscle mass and strength are the very essence of sarcopenia. Warning signs: it has become hard to rise from a chair without using your hands, your grip has weakened, your walking has slowed, and your arms and legs have "thinned" while the belly stays the same or grows.

How the test is done

Approximately — from grip strength (a dynamometer), walking speed, the chair-stand test and a muscle mass index (bioimpedance, DEXA).

Fat-free body mass is an indirect indicator of muscle mass — our body-composition calculators estimate it.

What affects the value

  • Strength training — the main and most powerful factor in preserving muscle.
  • Dietary protein — needed as building material, especially in older age.
  • Vitamin D — its deficiency worsens muscle function.
  • Hormones — a decline in testosterone and estrogen speeds up muscle loss.
  • Inactivity — even weeks of bed rest noticeably reduce muscle.

At what age does sarcopenia begin?

Slow muscle loss begins as early as after 30–40, but becomes noticeable after 50–60, especially in inactive people. The good news: strength training slows and partly reverses this process at any age.

How do I stop muscle loss?

The key is strength (resistance) training 2–3 times a week and enough protein in the diet. A normal vitamin D level, movement and treatment of chronic diseases also matter. This works even in older age.

How do I know if I'm losing muscle?
