Myoglobin — the muscle protein that stores oxygen

Myoglobin is a protein that stores oxygen in the muscles (functionally it is a "relative" of hemoglobin, but it lives inside muscle cells).

When muscle is damaged — skeletal or cardiac — myoglobin quickly enters the blood. It rises earlier than troponin, but is far less specific: it does not distinguish muscle from heart, so for diagnosing a heart attack it has today been largely replaced by troponin.

The reference range depends on the lab; approximate values:

Important: Because of its low specificity, an isolated myoglobin means little; for suspected heart attack troponin is used, while myoglobin points to the overall picture of muscle damage.

What an elevated level means

A high myoglobin means damage to muscle tissue — skeletal or cardiac.

Causes: myocardial infarction (an early rise), heavy exercise and trauma, rhabdomyolysis (massive muscle breakdown — dangerous for the kidneys), seizures, burns, some drugs. A sharply high myoglobin with dark urine is a reason to urgently rule out rhabdomyolysis.

What a low level means

A low myoglobin has no clinical significance — diagnostically only a rise matters. A normal level means there is no significant acute muscle damage.

How the test is done

Myoglobin is measured in blood (and, less often, in urine when rhabdomyolysis is suspected). It rises early but is quickly cleared by the kidneys, so its "window" of elevation is short.

For diagnosing a myocardial infarction, high-sensitivity troponin is used today — it is more specific. Myoglobin is more often used in the context of skeletal-muscle damage.

What affects the value

  • Myocardial infarction — an early rise in myoglobin.
  • Heavy exercise, trauma, seizures — raise it.
  • Rhabdomyolysis — raises it sharply (dangerous for the kidneys).
  • Intramuscular injections — may raise it.
  • Kidney disease — slows clearance and raises the level.
  • Some drugs (statins in myopathy) — raise it.

How does myoglobin differ from troponin?

Both enter the blood when muscle is damaged, but troponin is specific to the heart, whereas myoglobin is also in skeletal muscle. Myoglobin rises earlier but does not distinguish the heart from muscle, so troponin is used to diagnose a heart attack.

What is rhabdomyolysis?

Massive breakdown of muscle tissue, in which a lot of myoglobin is released into the blood. It can damage the kidneys, so a sharply high myoglobin, muscle pain and dark urine are a reason to seek help urgently.

Can myoglobin rise from exercise?

Yes. Intense or unaccustomed exercise damages muscle fibers, and myoglobin rises temporarily. This is usually harmless, but a marked rise with dark urine calls for ruling out rhabdomyolysis.
