Lymphocytes — the cells of adaptive immunity in the blood

Lymphocytes are cells of the white-cell differential responsible for "targeted", adaptive immunity: fighting viruses and immune memory.

Their proportion matters not only in infections: the lymphocyte percentage is part of the PhenoAge biological-age model — a fall in the proportion of lymphocytes is linked to weakening immunity and aging.

Both the proportion (%) and the absolute number are assessed; typical values:

Important: In children the proportion of lymphocytes is physiologically higher than in adults. It is best assessed by absolute number and over time.

What an elevated level means

A rise in lymphocytes (lymphocytosis) is a typical response to viral infections (respiratory infections, infectious mononucleosis, whooping cough, etc.). It is usually temporary and passes after recovery.

A persistent marked lymphocytosis, especially in older adults, needs attention: occasionally a blood disease lies behind it (for example, chronic lymphocytic leukemia).

What a low level means

A fall in lymphocytes (lymphopenia) occurs with acute stress and glucocorticoid use, severe and chronic infections, autoimmune diseases, after chemo- and radiotherapy, and with immunodeficiencies.

A persistently low proportion of lymphocytes is one of the signs of weakened immunity; in aging models it is regarded as an unfavorable factor.

How the test is done

Lymphocytes are measured in the differential count of the complete blood count. No special preparation is needed; acute stress before the draw can temporarily lower their proportion.

They are interpreted together with the total white-cell number and the rest of the differential, and when assessing inflammation — with neutrophils (the NLR index).

What affects the value

  • Viral infections — raise them.
  • Acute stress and glucocorticoids — lower them.
  • Chemo- and radiotherapy — lower them.
  • Autoimmune diseases — can lower them.
  • Age — the proportion of lymphocytes is higher in children.
  • Severe infections — can lower them.

Lymphocytes are high — what does it mean?

Most often it is a viral infection: lymphocytes are responsible for antiviral immunity. The rise is usually temporary. A persistent marked lymphocytosis, especially in older adults, needs a workup to rule out blood diseases.

Why are lymphocytes included in the biological-age calculation?

The proportion of lymphocytes reflects the state of immunity, and its weakening is part of biological aging. In the PhenoAge model the lymphocyte percentage is one of the blood parameters used to estimate biological age.

What do low lymphocytes indicate?

Lymphopenia occurs with acute stress, glucocorticoid use, severe infections, after chemo- and radiotherapy, and with immunodeficiencies. A persistent fall is a reason to sort out the cause together with a doctor.
