Holotranscobalamin is the "active" form of vitamin B12: the part bound to the carrier protein transcobalamin and actually available to the cells.
An ordinary total-B12 test measures all the vitamin, including the inactive fraction, and so can "miss" an early deficiency. Holotranscobalamin shows precisely the working store of B12 and is regarded as a more sensitive marker.

What an elevated level means
A high holotranscobalamin usually has no clinical significance and is most often due to taking B12 supplements or injections.
A rise in active B12 on its own is not harmful; it needs no separate treatment.
What a low level means
A low holotranscobalamin is an early sign of vitamin B12 deficiency, before total B12 even falls.
Causes: a dietary lack (veganism), impaired absorption (atrophic gastritis, absence of intrinsic factor, bowel disease), taking metformin and acid-lowering drugs, older age. Deficiency shows itself as anemia (with a rise in MCV), fatigue and neurological symptoms.
How the test is done
Holotranscobalamin is measured in blood from a vein. It is ordered when the B12 status needs clarifying — as a more sensitive test than total vitamin B12.
