Diastolic function is the ability of the left ventricle to relax and fill with blood during diastole at normal pressure. When the heart becomes stiff and fills worse, this is called diastolic dysfunction.
It is assessed from echocardiography data: the E/A ratio, tissue Doppler e′, left atrial volume and other parameters.
Why it matters
Diastolic dysfunction is a common cause of shortness of breath and heart failure with preserved ejection fraction, where the ejection fraction is normal yet the heart still works poorly. With age the myocardium naturally becomes stiffer, so diastolic function is a sensitive marker of heart aging.
How it is assessed
Key measures: the E/A ratio of transmitral flow, the e′ velocity on tissue Doppler (reduced in dysfunction), the E/e′ ratio (reflects filling pressure), left atrial volume and the tricuspid regurgitation velocity. No single measure is used in isolation.
What to do
There is no specific 'pill' — it is important to treat the causes: control blood pressure, blood sugar and weight, treat left ventricular hypertrophy, and maintain regular aerobic activity. Exercise helps preserve the elasticity of the myocardium.

Is grade 1 diastolic dysfunction dangerous?
Grade one (impaired relaxation) is very common with age and often runs without symptoms. It is a reason to control blood pressure and risk factors, not a diagnosis of heart failure.
Can diastolic function be restored?
Age-related changes cannot be fully reversed, but controlling blood pressure, weight and blood sugar and regular aerobic exercise slow progression and reduce symptoms.
